Community Health Centers
CMS has finalized updates to how Rural Health Clinics (RHCs) and Federally Qualified Health Centers (FQHCs) report care coordination services. Instead of using a single HCPCS code (G0511), you will now report individual CPT and HCPCS codes for better payment accuracy.
As of January 1st, 2025, FQHCs and RHCs are able to utilize the following codes:
Remote Patient Monitoring
99453, 99454, 99457, 99458
Chronic Care Management
99490, 99439
