WHAT INFORMATION ABOUT ME IS COLLECTED?

Primarily, the collection of your Personal Information assists us in securely tracking, managing, and transmitting your biometric data to your healthcare provider to help track and manage your health. We will have an agreement, such as a BAA, with your healthcare provider(s) limiting our use of such data, which controls over this policy.

ANONYMOUS INFORMATION

Anonymous Information is information, any single item of which, by itself, cannot be used to identify or contact you, including demographic information (such as age, profession, gender, current location, zip code, birth date, or year of birth), IP addresses, browser types, domain names, and data involving the use of RemetricHealth Services, device, and data services. Certain Anonymous Information may be considered a part of your Personal Information if it is combined with other identifiers (for example, combining your zip code with your street address) in a way that enables you to be identified. But the same pieces of information are considered Anonymous Information when they are taken alone or combined only with other Anonymous Information (for example, your viewing preferences).

USING REMETRICHEALTH SERVICES

Information is collected when you interact or communicate with us by phone, email, or text message, or other forms of communication.  Information is also collected through our online and mobile websites, services, devices, and applications that we own or operate. When you register to use RemetricHealth Services, you will be required to provide us with information (including Personal Information and Anonymous Information) such as name, postal address, e-mail address, and birth date. We may also receive Personal Information about you from third parties as part of the registration process, such as from health and care providers who, with your consent, share your Personal Information in connection with our performing our services.

COOKIES AND ACTION TAGS

We may collect Anonymous Information passively using “cookies” and “action tags” when you use the Site.

WHAT DOES REMETRICHEALTH DO WITH THE INFORMATION IT COLLECTS?

In general, we use the information collected to provide RemetricHealth Services to you to securely track, manage, and transmit your Personal Health Information to your healthcare provider to help track and manage your health.  It may also be used to help us understand the use and utilization of

RemetricHealth Services, for internal operations such as operating and improving RemetricHealth Services, to contact you for customer service purposes, and to perform data analysis using Anonymous Information and aggregated data.

RemetricHealth may use your information to send you a welcoming e-mail that may confirm your user name and password. If you identify yourself to us by sending us an e-mail with questions or comments, we may use your information (including personally identifiable information) to respond to your questions or comments, and we may file your questions or comments (with your information) for future reference. We may also use the information collected to send announcements and updates regarding RemetricHealth Services or, if applicable, about your billing account status.

DOES REMETRICHEALTH COLLECT INFORMATION FROM CHILDREN?

We are committed to protecting the privacy of children. Our goal is to comply with laws and regulations relating to the collection and use of information from children, including the Children’s Online Privacy Protection Act (COPPA), if and to the extent such laws may be applicable to the operation of this website. COPPA requires that we inform parents and legal guardians about how we collect, use, and disclose personal information from minors under 13 years of age. In order for a child under 18 to use the Services, a parent must register the child. As part of the registration process, a parent will be asked to provide both the parent’s and the child’s e-mail address (for those children 10 years of age and older). For children ages 10 to 18, we will send communications to both the child and the parent. A parent will have access to any information contained on the Portal of a child he/she has registered. At any time, a parent can change or delete the Personal Information we have collected from his or her child by contacting us at customercare@RemetricHealth.com. Depending on the scope of a parent’s request, it may be necessary to delete a child’s registration completely, in which case the child would no longer be eligible receive Services.

CAN I MODIFY MY PERSONAL INFORMATION?

As a User, you may log onto your profile or contact us at customercare@RemetricHealth.com to modify your Personal Information. To protect your privacy and security, we may take reasonable steps (such as requesting a unique password) to verify your identity before granting you profile access or making corrections. You are responsible for maintaining the secrecy of your unique password and account information at all times.
